Summary of Non-exempt Employee Benefits – Magic Valley

 Employment status (hours worked per two week period and as regularly scheduled):

Full-time = 72 - 80 hours   Part-time III = 56 – 71 hours

Part-time II = 32 - 55 hours

Who Pays

Group Health Insurance Plan:  Medical & Prescription, Dental, Vision, Orthodontics

Newhire:  FT, PT II & III employees. If enrolled within 30 calendar days of hire, insurance becomes effective the first day of the month following hire date.  Caution:  Only other time to enroll in plan is during annual Open Enrollment or if a change in family status occurs.

Three separate medical and two separate dental programs are administered through Ameriben and Delta Dental.  The Managed Health Care plans are designed to meet the varying needs and budgets of employees with available features such as a prescription program, vision care, orthodontics, and preventative care.

25% Discount on St. Luke’s Facility Fees

Benefit eligible employees only (32 – 80 regularly scheduled hours per pay period).  Discount available to employee and eligible dependents. Effective date of hire.

After insurance has paid, benefit eligible employees and eligible dependents receive a 25% discount off the balance of the facility portion of their bill for covered services.  Must be a St. Luke’s facility.  Does not apply to physician services.

Long Term Disability

FT and PT III (56 – 80 hours per pay period).  Effective first of the month following 90 days of employment.

Provides 60% of base earnings after a 90-day waiting period (from day of disability) for own occupation disability.  Maximum benefit per month is $8000.

Group Term   Life Insurance

Benefit eligible employees (32 – 80 hours per pay period). 

Effective 1st of the month following 90 days of employment

Benefits are paid at 1x your annual salary for full-time employees.  Reduced benefit formula for part time employees depending on FTE.  Double indemnity for accidental death.

Supplemental Term Life Insurance and/or Supplemental AD&D

Benefit eligible employees (32 – 80 hours per pay period).

Employee - Guarantee issue of up to $120,000 with a maximum of $300,000 with evidence of insurability. Spouse – Guarantee issue of $10,000 with a max of $100,000 with evidence of insurability.  $5,000 or $10,000 available for dependent children if employee has supplemental coverage.

Retirement Program

Eligible for match after 1 year of service and 1,000 hours

 

403(b) Defined Contribution Matching Plan.  Employee contributes a percentage of gross earnings.  After one year employment and at least 1,000 hours worked, employer matches up to a maximum of 4.5%.  100% vested after 6 years of eligible service (1,000 hours work per year).

Flexible Spending Accounts

Benefits eligible employees (32 – 80 hours per pay period).  First day of the month following 90 days of employment.

Set aside pre-tax earnings for out-of-pocket health care and dependent care expenses.  Yearly maximums:

  • Medical, Dental, Vision:  $5,000.
  • Dependent Care:  $5,000.

Paid Time Off (PTO)

Benefit eligible employees (32 – 80 hours per pay period).  Accrues from date of hire.  May be used the first pay period following 90 days employment.

 

Used for vacation, holiday pay and short-term illness or injury.  One year full-time accrual is 184 hours (23 days). Accrual for 32 – 79 hours per pay period is pro-rated based on actual hours paid.   Increased accrual rate after 4 and 9 years of employment.  Maximum accrual = 320 hours.

Holidays

Time and one-half for working a recognized holiday:  New Year’s Day, Memorial Day, 4th of July, Labor Day, Thanksgiving and Christmas.  Double time if called in on holiday and not scheduled.  90- day requirement does not apply to hours worked on a holiday.

Extended Sick Leave (ESL)

Same as PTO (see Paid Time Off).  Depending on ESL balance, predetermined PTO hours must be used before qualifying for ESL; if hospitalized, ESL is effective without PTO wait period

To be used for an eligible employee’s own injury or illness.  Hours accrue at the rate of 64 per year for 80-hour full-time employees.  Accrual for 32 – 79 hours per pay period is pro-rated based on actual hours paid.  Maximum accrual is 520 hours.  Annual 8 hour bonus if at 520 hours.

(6:00 p.m. to 6:00 a.m.): Employees required to work any hours during this twelve (12) hour period receive a differential of 12% of their pay grade minimum or $1.75 per hour whichever is greater.

8% of pay grade minimum or $.75 per hour whichever is greater for every hour worked between 11:00 p.m. Friday and 11:00 p.m. Sunday

Blood Risk Profile, Mammograms for female employees and wives of male employees at age 40 (free – certificates issued during birth month); Healthy Pregnancy Program; wellness events and other wellness lab tests as announced (i.e., thyroid test).

Benefits eligible employees and eligible dependents.  Effective immediately.

Confidential, short-term, professional counseling service for employees and family members. (some services subject to discounted fees)

Voluntary  Insurances  Benefits

First of month following 90 days employment.

Nationally recognized companies providing a variety of insurance products at group discount rates, i.e., cancer, long term care, home and car, pet, whole life.

Pay in Lieu of Benefits (PILB)

Employees working 32 – 80 hours per pay period may elect PILB.  Must be elected within 30 calendar days of hire, becomes effective the first pay period following receipt of election form.  Only other time to enroll on this plan is during open enrollment or due to a change in family status.

PILB is an optional benefit election designed to allow benefits eligible employees who have health insurance coverage under a non-St. Luke’s health plan to elect a higher rate of pay in lieu of certain benefits.  If you choose PILB, you agree to forfeit: Group health plan, PTO, PTO donations, ESL, ESL Bonus, LTD, Group Life/Supplemental Life Insurance, Adoption reimbursement.

FT (80 hours per pay period): 15%

PT - III (56-79 hours per pp):   12%

PT - II (32-55 hours per pp):      9%

Tuition / Certification

Reimbursement & Scholarships

All employees.  Effective first semester following 90 days of employment.  (see entire policy for full instructions and approvals)

Tuition/Certification assistance for work-related classes and/or degree programs.  School year maximum is $3,000 for full-time employee; pro-rated for part-time employees.  Limited scholarships available for hard-to-recruit positions.

Adoption Reimbursement

Benefits eligible employees (32 – 80 hours work per pay period).  After one year of employment.

Reimbursement up to $1,500 is available for expenses incurred when adopting a child.
